Can an Old Injury Cause Pain Years Later?

Have you ever injured your back, knee, ankle or shoulder years ago, only to find that the same area starts hurting again?
It can be frustrating when an injury you thought you'd recovered from suddenly seems to return. But yes, an old injury can sometimes contribute to pain years later — although it isn't necessarily the original injury that is causing the problem.
Why can an old injury cause problems?
After an injury, the body adapts. You may unconsciously change the way you move, put more load through another area or avoid certain movements because they previously caused pain.
These changes can become habitual, even after the original injury has healed.
For example, an old ankle injury may affect how you walk or run. Over time, this could alter the load placed on your knee, hip or lower back.
Previous injuries can also leave you with reduced strength, mobility or confidence in certain movements, making the area less able to tolerate increases in activity.
Does pain mean you've damaged the area again?
Not necessarily.
Pain doesn't always mean that you've caused new tissue damage. Your symptoms can be influenced by factors such as activity levels, sleep, stress, training load and how sensitive the affected area has become.
This is why simply resting every time an old injury becomes painful isn't always the best long-term solution.

Why does an old injury suddenly hurt again?
Sometimes the trigger is surprisingly simple.
You may have:
Increased your exercise or running
Started a new sport
Spent more time sitting or travelling
Changed your training programme
Reduced your strength training
Returned to activity after a period of inactivity
The tissues may simply be experiencing more load than they're currently conditioned to handle.
What can you do?
The most useful approach is often to understand why the area has become painful again, rather than focusing solely on the original injury.
An assessment can look at your movement, strength, mobility, previous injury history and current activity levels. From there, treatment and a progressive exercise programme can help rebuild your capacity and confidence.
