Despite its name, shockwave therapy doesn't involve electric shocks. Instead, it uses high-energy acoustic (sound) waves to stimulate the body's natural healing processes.

But how does it actually help ease muscle tension?

The answer is more complex than simply "loosening tight muscles". Radial shockwave therapy appears to influence the muscles, connective tissues and nervous system in several ways, helping to reduce pain and improve movement.


What Is Radial Shockwave Therapy?

Radial shockwave therapy uses a handheld device to deliver pulses of acoustic energy through the skin into the affected area.

Unlike focused shockwave therapy, which targets deeper tissues with greater precision, radial shockwave therapy spreads the energy over a wider area. This makes it particularly useful for treating larger muscle groups, tendons and areas of persistent muscular tightness.

Treatment usually lasts only a few minutes and is often combined with exercise rehabilitation and hands-on treatment where appropriate.


Muscle Tension Isn't Always the Problem

Many people describe feeling that their muscles are "tight", but tightness isn't always caused by muscles physically shortening.

Sometimes muscles feel tight because:

  • They're working harder to compensate for weakness elsewhere.

  • The nervous system is increasing muscle tone in response to pain.

The acoustic waves appear to influence pain-signalling nerves, making the treated area less sensitive. When pain decreases, muscles may naturally relax because the body no longer feels the need to protect the injured area.

For many people, this makes movement feel easier and more comfortable.

It Improves Blood Flow

Shockwave therapy stimulates local circulation, increasing blood flow to the treated tissues.

Improved circulation helps deliver oxygen and nutrients while removing waste products produced during normal metabolism. Although increased blood flow isn't a cure on its own, it supports the body's natural repair processes.

It Stimulates Tissue Healing

In chronic injuries, the body's healing response may have slowed or become less effective.

Shockwave therapy creates controlled mechanical stimulation within the tissues, encouraging cellular activity that supports repair. This is one reason it's commonly used for long-standing tendon problems such as Achilles tendinopathy, plantar fasciopathy and tennis elbow.

While muscle tension may improve, treatment is often addressing the underlying tissue rather than simply the symptom.

Why Exercise Still Matters

Shockwave therapy is not a standalone solution.

If muscle tension has developed because of weakness, poor movement patterns or excessive training load, those factors still need to be addressed.

Your treatment plan may also include:

  • Strengthening exercises.

  • Mobility work.

  • Load management.

  • Running or sports-specific rehabilitation.

  • Advice on training progression.

Combining shockwave therapy with an active rehabilitation programme generally offers better long-term outcomes than passive treatment alone.


What Does Treatment Feel Like?

During treatment, you'll feel a series of rapid tapping sensations over the affected area.

Some areas may feel slightly uncomfortable, particularly if they're already painful, but the intensity can usually be adjusted to keep treatment tolerable.

The session typically lasts between five and ten minutes, depending on the condition being treated.

Afterwards, you may experience mild soreness for a day or two, similar to the feeling after a challenging workout. This usually settles without any specific treatment.


Is Shockwave Therapy Suitable for Everyone?

Radial shockwave therapy may be appropriate for a range of persistent musculoskeletal conditions, but it isn't suitable for everyone.

It may not be recommended if you:

  • Are pregnant (over the treatment area).

  • Have certain bleeding disorders or are taking some blood-thinning medications.

  • Have an active infection in the treatment area.

  • Have certain tumours affecting the area to be treated.

  • Are treating an area where bone has not fully matured (in children and some adolescents).
