Why Quadriceps Strength Matters for Knee Injuries

When people injure their knee, the focus is often on the painful structure — the meniscus, ligament, tendon or joint. But one of the most important factors influencing recovery is often the strength and function of the quadriceps muscles.

The quadriceps are the large muscles at the front of the thigh. They play a crucial role in straightening the knee, absorbing forces during activities such as walking, running and landing, and controlling movement when the knee bends.

Research consistently shows that reduced quadriceps strength is common following knee injury and can have important consequences for recovery.


Why does quadriceps strength decrease after injury?

Knee pain and swelling can cause a phenomenon called arthrogenic muscle inhibition, where the nervous system reduces the ability of the quadriceps to contract normally. This can happen surprisingly quickly following an injury.

The result can be muscle wasting, reduced strength and poorer control of the knee. This creates a difficult cycle:

Pain/swelling → reduced quadriceps activation → weakness → reduced knee function → greater difficulty loading the knee.

Following ACL reconstruction, for example, quadriceps weakness can persist for years. A systematic review found that the injured leg remained weaker than matched healthy controls for up to four years after surgery.


Why Quadriceps Strength Matters for Knee Injuries

Stronger quadriceps help the knee cope with load

The quadriceps don't simply make the knee stronger. They help the entire lower limb tolerate and control load.

Every time you walk downstairs, squat, run, jump or land, the quadriceps work to control knee flexion and absorb force.

This is particularly important when returning to sport. A knee may feel relatively comfortable during everyday activities but still lack the strength and capacity required for running, changing direction or jumping.

That is why pain reduction alone does not necessarily mean the knee is ready to return to full activity.


Quadriceps strength and patellofemoral pain

There is also evidence linking quadriceps weakness with patellofemoral pain — commonly experienced as pain around or behind the kneecap.

A systematic review and meta-analysis found moderate evidence that quadriceps weakness is a risk factor for developing patellofemoral pain.

Current best-practice guidance therefore recommends knee-targeted exercise therapy, often incorporating quadriceps strengthening, as a primary treatment for patellofemoral pain.


What about knee osteoarthritis?

Quadriceps strength is also important in people with knee osteoarthritis.

An updated systematic review involving more than 46,000 people found that knee extensor weakness was associated with the development of knee osteoarthritis, suggesting that maintaining or improving quadriceps strength may have a protective role.

Once osteoarthritis is established, strengthening remains one of the best-supported forms of exercise treatment. A 2025 review of randomised controlled trials found that quadriceps strengthening significantly improved pain and function in people with knee osteoarthritis.


How should you strengthen the quadriceps?

There is no single "best" quadriceps exercise. The right exercise depends on the injury, symptoms, stage of rehabilitation and the individual's goals.

Depending on the situation, rehabilitation may include:

  • Isometric exercises – useful when pain or irritability is high

  • Straight-leg raises – particularly during early rehabilitation

  • Squats and sit-to-stands

  • Step-ups and step-downs

  • Split squats and lunges

  • Leg press exercises

  • Knee extensions

  • Progressive resistance training

  • Running, jumping and sport-specific exercises during later rehabilitation

Importantly, strengthening should be progressive. As the knee becomes more tolerant, the muscles need to be challenged with sufficient resistance to stimulate further improvements in strength and capacity.

Research following ACL reconstruction demonstrates that structured strengthening can significantly improve quadriceps strength and functional performance, although improvements in pain are less consistent.


Strength is only one part of rehabilitation

Although quadriceps strength is extremely important, it is not the whole story.

A successful knee rehabilitation programme should also consider:

Mobility → strength → balance → coordination → load tolerance → sport/activity-specific capacity.

The aim isn't simply to build a bigger quadriceps muscle. It is to restore the knee's ability to produce and control force confidently and repeatedly.

For athletes, this may ultimately mean being able to sprint, decelerate, jump and change direction without significant differences between legs.


Don't wait until the pain disappears

One of the biggest mistakes after a knee injury is waiting for the knee to feel completely normal before starting rehabilitation.

In many cases, appropriately prescribed strengthening can begin relatively early, with the exercise selected and modified according to pain, swelling, injury type and irritability.

The goal is not to "push through" significant pain. It is to gradually expose the knee to increasing levels of load so that strength and capacity can be rebuilt.
