MRI scans are incredibly useful tools, but they are not a complete picture of your injury. They are one part of the assessment process—not the whole answer.

What Is an MRI Scan?
Magnetic resonance imaging (MRI) uses powerful magnets and radio waves to create detailed images of structures inside the body.
Unlike an X-ray, which is mainly used to look at bones, an MRI can provide detailed images of soft tissues such as:

Ligaments.
Cartilage.
Discs in the spine.
Some areas of bone.
Because of this level of detail, MRI scans are commonly used when investigating sports injuries and persistent pain.
What Can an MRI Show?
An MRI can be extremely helpful for identifying certain structural changes, including:
Muscle tears.
Ligament injuries.
Tendon damage.
Cartilage injuries.
Disc problems.
Bone stress injuries.
Swelling or inflammation.
For some injuries, an MRI provides valuable information that helps guide treatment decisions.

Why Doesn't an MRI Always Explain Your Pain?
One of the biggest misunderstandings about MRI scans is assuming that everything seen on a scan is the cause of symptoms.
The reality is more complicated.
Many people have changes visible on MRI scans despite having little or no pain.

Disc bulges in people without back pain.
Tendon changes in people who are still able to exercise comfortably.

Why Do Some People Have a "Normal" MRI but Still Have Pain?
A normal MRI can sometimes feel frustrating, especially when you're experiencing real symptoms.
However, pain does not always come from visible tissue damage. The nervous system plays an important role in how the body detects and responds to threats.
Pain can be influenced by:
Sensitivity of the nervous system.
Previous injury.
Changes in movement.
Load exceeding your body's current capacity.
Stress and recovery levels.
A normal scan can actually be useful information because it may help rule out certain serious problems and allow focus on rehabilitation.
Why Do Some People Have Abnormal MRI Findings Without Pain?
The opposite can also happen.
A scan may show changes that sound concerning but are actually common in people who are active and symptom-free.
For example, an athlete may have minor tendon changes on an MRI but still perform at a high level without pain.
This is why scan findings must always be interpreted alongside:
Your symptoms.
Your injury history.
Your physical examination.
Your goals.
When Is an MRI Useful?
MRI scans can be particularly valuable when:
A serious injury is suspected.
Symptoms are not improving with appropriate rehabilitation.
There is uncertainty about the diagnosis.
Surgical decisions need to be considered.
A clinician needs more information about a specific structure.
However, an MRI is not always needed for every sports injury. Many common conditions can be diagnosed and managed effectively through a detailed history and physical assessment.
The Importance of Clinical Assessment
A good assessment looks at the whole picture:
What happened when the injury occurred?
Where is the pain?
What movements make it better or worse?
How does your body move?
What demands does your sport place on you?
What factors may have contributed?
This information helps create a treatment plan that addresses the cause of the problem, not just the image on a scan.
